Objectives

The objective of the Endocrinology section of the course is to provide to medical students the appropriate pathophsysiolocal and clinical basis needed to:
-Critically evaluate and perform pertinent correlations of clinical symptoms and objective signs with functional alterations and pathogenetic mechanisms of the main endocrine and metabolic diseases (see Program)
-Put forward a diagnostic hypothesis among the main endocrine and metabolic diseases through a correct clinical, laboratory and instrumental approach
-Be aware or the epidemiological and socioeconomic relevance of the main endocrine and metabolic diseases
The objective of the Gastroenterology section is to provide to medical students the specific knowledge in the context of the main gastrointestinal diseases, with particular reference to:
-the management of the patient with these diseases, raising his awareness on the importance of the prevention of several diseases of the digestive tract (gastric cancer, colorectal cancer, hepatocellular carcinoma) not only by monitoring risk groups but also by screening of the general population
The objective of the internship section is to provide medical students with adequate practical support to their theoretical knowledge. To this purpose, small group ofstudents will spend a period of time (two weeks) working in strict association with physicians busy in the daily clinical activity in the wards and out-patient clinics of the Endocrinology-Diabetes and Gastroenterology Units of Cagliari University Hospital

Prerequisites

Having attended no less than 70% of the total lectures and not less than 50% of each module and attendance of all hours scheduled for the professional activity

Contents

Endocrinology section
Pathopysiology, clinical aspects and general therapeutic principles of the following diseases (selected on the basis of prevalence and potential impact on the general):
• Endocrine regulation and feed-back.
• Diseases of anterior and posterior pituitary.
• Hyperthyroidism, hypothyroidism and autoimmune thyroid diseases.
• Diffuse and nodular goiter and thyroid neoplasia.
• Disorder of mineral metabolism and parathyroid diseases .
• Adrenal cortex diseases.
• Adrenal medulla disease.
• Polyendocrine autoimmune syndromes.
• Pathology of the female and male reproductive axis.
• Endocrinology and cancer.
• Endocrinology of weigth balance.
• Diabetes Mellitus and its complications. Dibetic foot.
• Ipoglicemia
• Disorders of lipid metabolism
• Obesity and metabolic syndrome.
• Nutritional aspects of endocrine and metabolic diseases
• Laboratory algorithms in the diagnosis of endocrine and metabolic diseases

Gastroenterology section
Pathopysiology, clinical aspects and general therapeutic principles of the following diseases (selected on the basis of prevalence and potential impact on the general):
• Gastroesophageal reflux disease.
• Tumors of the esophagus.
• Gastric infection by Helicobacter pylori.
• Gastritis.
• Tumors of the stomach.
• Functional diseases according to Rome III:
-irritable bowel syndrome
-Dyspepsia (Epigastric pain syndrome, postprandial distress syndrome, nausea, vomiting, rumination, belching).
• Celiac disease.
• The short bowel syndrome.
• Pancreatitis acute and chronic.
• Tumors of the pancreas.
• The neuroendocrine tumors.
• Tumors of the colon and rectum.
• Crohn's disease.
• Ulcerative colitis.
• Diverticular disease.
• Diarrhea acute and chronic.
• Constipation.
• The digestive bleeding.
• Hepatic steatosis and alcoholic hepatitis.
• Non-alcoholic steatoepatitis
• Hemocromatosis and Wilson disease.
• Autoimmune hepatitis.
• Cholostatic syndrome.
• Viral hepatitis.
• Cirrhosis.
• The hepatocellular carcinoma.

Teaching Methods

Theoretical lessons and practice in wards, outpatient clinics and laboratories

Verification of learning

-Written examination (multiple answers test) at the first session (February) for students attending regular courses. After successfully passing the written test, students willing improve the mark received, may later pass an oral examination at one of the next sessions scheduled for regular students.
-All other subsequent sessions for regular and failing students will consist in an oral examination
The mark of the written test will be expressed in thirtieths/30 and will depend upon the number of right answers according to a a pre-arranged table. The mark of the oral examination, also expressed in thirtieths/30, will be based upon the knowledge degree, the ability of elaborate a diagnostic argument on the basis of clinical signs and symptoms, biochemical test and instrumental investigations and the verbal capacity in the explanation.
